Community Nurse (Community Nursing)

About the role

The Community Nurse will join NHS Greater Glasgow and Clyde’s community health team, providing high‑quality nursing care to patients in the community. Working closely with the District Nurse, you will manage a caseload of patients, delivering assessment, planning, implementation and evaluation of care programmes.

Key responsibilities

• Assess, plan and implement individualised care programmes in partnership with patients, families and carers.
• Manage a caseload of community patients and support the District Nurse in day‑to‑day operations.
• Assume responsibility for the caseload during short‑term absences of colleagues.
• Ensure compliance with local, regional and national policies, procedures and guidelines.
• Provide autonomous, evidence‑based nursing interventions considering cultural, gender and lifestyle factors.

Required profile

• Current registration with the Nursing and Midwifery Council (NMC) and a valid NMC PIN.
• Membership of Disclosure Scotland’s PVG scheme.
• Ability to work full‑time (36 hours per week) on a rotating shift pattern, including weekends, public holidays and out‑of‑office hours.
• Mid‑level experience (approximately 2 years) in community or district nursing.
